questions about health insurance and help you through the application process.

claim?
A. Call USBenefit Plan at (888) 316-2795 Monday through Thursday from 7:00 am to
6:00 pm Central time and Friday 7:00 am to 3:00 pm Central time. We can locate any
claim forms, explain the process, and assist you with filing your claim. We are
